Whether you need to request a class change, submit a withdrawal, opt out of an upcoming performance, or complete a program-specific form, you’ll find the appropriate forms and deadlines here. Please review the information provided with each form carefully before submitting. Submitting a form does not automatically guarantee approval or confirm that a requested change has been processed. Our administrative team will follow up when confirmation is required.
Class Change Request Form
Use the Class Change Request Form to transfer from one class to another or drop an individual class while remaining enrolled at NAOD.
All class transfers are subject to availability, age/level requirements, and studio approval. Approved transfers submitted outside of NAOD’s complimentary transfer period are subject to a $15 non-refundable transfer fee.
Please note that dropping or transferring a class does not result in a refund or credit for tuition already paid or due for the current tuition installment. Any applicable tuition adjustment will take effect with the next scheduled installment. Previously assessed non-refundable costume, concert, competition, or other fees will remain on the family account.

Withdrawal Form
Use the Withdrawal Form when a dancer will be discontinuing all enrolled classes and fully withdrawing from Norma’s Academy of Dance.
Withdrawal requests must be submitted online through the Withdrawal Form. NAOD requires notice by the 15th of the month prior to the next tuition billing cycle on the 1st of the month. Tuition already paid or due is non-refundable, and non-attendance does not constitute withdrawal or cancel future tuition obligations.
Previously assessed non-refundable costume, concert, competition, or other fees remain the responsibility of the family and are not canceled by withdrawing from the studio.
Important: If your dancer is remaining enrolled at NAOD and you only wish to drop one class or transfer to another class, please use the Class Change Request Form instead. Verbal, email, or text withdrawal requests are not accepted.
Holiday Concert Opt-Out Form
Being a part of this special performance is an incredible opportunity for students to showcase their growth, build confidence, and celebrate their achievements with family and friends. Participation in the Holiday Concert is an essential component of their dance education, and we can’t wait to see them shine on stage! However, we understand that not all families may wish to participate in our upcoming Holiday Concert. If you choose not to have your child participate, please fill out the following form by September 4th.
